What Is the BALANCE Model?
The Beneficiary Access to Lifechanging Anti-obesity and Nutrition Care Explored (BALANCE) Model is a CMS Innovation Center program that adds obesity medication coverage to Medicare Part D. It runs from July 1, 2026 through at least December 2027.
Under BALANCE, participating Part D plans will cover FDA-approved anti-obesity medications — including semaglutide (Wegovy) and tirzepatide (Zepbound) — with a patient copay capped at $50 per month.
Who Qualifies
Medicare Part D enrollees who meet these criteria:
- BMI ≥ 30 (obese), or BMI ≥ 27 with at least one weight-related comorbidity (type 2 diabetes, hypertension, dyslipidemia, cardiovascular disease, sleep apnea)
- Enrolled in a participating Part D plan (not all plans are participating — check with your plan)
- Prescription from a qualified provider
Important distinction: This covers FDA-approved brand-name medications only. Compounded semaglutide and tirzepatide are not eligible under BALANCE.
